Accessing the IH network for partner agencies and service providers
External agencies
External agencies include private residential facilities, educational institutions, private physician offices/clinics, and government agencies which require access to the IH network typically for client care or educational purposes. Click on the appropriate link below for forms required to request access for:
External Access Letter of Agreement: The manager of the physician office/clinic completes the External Access Agreement form on behalf of their organziation. This form is only required to be submitted once for the office/clinic.
Medical Office Assistant (MOA) Access Agreement: to be completed only for Medical Office Assistants (MOA) requiring access to the IH network. MOA access will be reviewed on an annual basis and may require an updated access agreement to be submitted.
External Access Letter of Agreement: The manager/director of the external agency completes the External Access Agreement form on behalf of their organziation. This form is only required to be submitted once for the external agency.
External User Access Agreement: This form is required for each individual (excluding Medical Office Assistants - see Private Physician Office/Clinic requirements below) at the external agency that requires access to the IH network (i.e. student at the educational facility, employee at the private residential facility or government agency). This is a two-sided form which provides instructions on the back. Note that this is also the form you will use to notify IH of any termination of access.
To avoid processing delays, please ensure that all required forms have been completed with required information and signatures prior to their submission.
Service providers
Service providers include those external entities that require access to the Interior Health network to provide support services to the IMIT (Information Management & Information Technology) infrastructure and software products. A contract between the service provider and Interior Health must be in place prior to any request for access being authorized.
External Access Letter of Agreement: This agreement form is completed and signed by the service provider's CEO/Vice President/President/Director on behalf of their organziation. This form is only required to be submitted once for the service provider company.
Service Provider Access Agreement: This agreement form is completed and signed by the service provider's CEO/Vice President/President/Director on behalf of their organziation. This form is only required to be submitted once for the service provider company. Upon approval a single service provider account is setup to be utilized by all the service provider's staff that provide support services remotely to IH.
