Breathe Well is an at home program that helps patients being discharged from hospital to manage their Chronic Obstructive Pulmonary Disease (COPD). It is meant to provide intensive home-based respiratory care, education, and case management to address the causes that have led to an acute care visit, including Cardio Respiratory Home assessments, COPD Flare up Plans, ongoing education and home based activity programs; Inhaled Medication Education and review, as well as Tobacco Dependency Counseling. This service is available Monday through Friday, 8:00 am to 4:00 pm.
