Short-Stay Service: Convalescent Care
Convalescent care is a short-term care service primarily for older adults with complex medical needs who require more time to regain strength and mobility before returning home, in order to manage their personal hygiene, grooming and overall physical wellbeing.
To help you transition back to your community, you'll be expected to actively participate in recovery, including goal setting, personal care and mobility. Your care team will work with you to develop a plan to help you achieve your recovery goal. The length of stay in convalescent care is dependent upon your recovery, and is expected to be approximately four to eight weeks.
During Your Stay
Your convalescent care team will meet regularly to plan for your needs and progress. This team is made up of a variety of health-care professionals and may include physiotherapists, occupational therapists, rehabilitation assistants, social workers and nursing staff.
- List of current medications
- Provincial Health Number (PHN)
- Emergency contact information
- Washable clothes
- Ambulatory items (e.g., wheelchairs, walkers, canes, walking shoes)
- Personal items (e.g., hearing aids, dentures, toiletries)
- Other items (e.g., books, calendar, a framed photo or other memento, cell phone, address book, cheque book or pocket money)
Cost
If you receive publicly subsidized convalescent care, you will pay a fixed daily rate as per cost for short-stay services.
If you or your family believes you will experience serious financial hardship by paying this rate, you may apply for a temporary reduction. For more information or to apply for a temporary reduction, contact your Case Manager and/or your local Home Health and Community Care Office.
