Hospital at Home
An innovative health-care model that enables hospital-level care to be provided in the comfort of a patient's home. You remain a hospital patient and your hospital ward is your own home. We bring the hospital to you!

Hospital at Home (HaH) overview
HaH aims to improve the patient, caregiver and health-care provider experience of care. It's an internationally recognized model of care, operating for over 20 years. Programs exist in other countries, and in other B.C. health authorities.
- Patients receive 24/7 care through a combination of virtual and in-person visits to the home
- Vital sign readings are taken in the home and communicated electronically to the care team
- All required technology and medications are provided
- There is no cost to patients on the program
Hear patient and staff experiences from Vancouver Coastal Health and Providence Health Care’s HaH program:
A consensus definition on Hospital at Home was presented at the World Hospital at Home Congress in 2023 by Professor Michael Montalto, on behalf of the Scientific Committee:
"Hospital at Home/ Hospital in the Home (HaH/ HITH) is an acute clinical service that takes staff, equipment, technologies, medication and skills usually provided in hospitals and delivers that hospital care to selected people in their homes or in nursing homes. It substitutes for acute inpatient hospital care.
It’s goal is to improve the lives of sick people who need hospitals, by changing the culture of hospitals to deliver hospital-level care at home."
