Is a referral required?
Are there eligibility restrictions?
To receive home rehabilitation services, clients must meet standard health care eligibility requirements and require care following discharge from a hospital, care at home rather than hospitalization, or care because of a terminal illness.
Is there a cost?
Possibly. Additional charges may be applied for associated supplies and services required for nursing care, ongoing home support and adult day programs. Visit the costs page to learn more. |
