About East Kootenay Heart Function Clinic

The East Kootenay Area Heart Function Clinic (HFC) is an outpatient clinic available to support patients who live in the East Kootenay region with a diagnosis of heart failure. Clients are supported with heart failure self-management education, tele-home monitoring, lifestyle behaviour education to help reduce risks, medical care, medication adjustments and educational information.
