Provides subacute care following a period of hospitalization; for the older adult with a chronic illness requiring further time to recover prior to returning home, where you will receive nursing and rehabilitation services from a team of health care professionals. This publicly subsidized activation program will help you regain the strength and mobility you need for daily tasks and activities once you’ve returned home.
