The Post-Heart Transplant Clinic follows patients after a heart transplant and helps to bring their ongoing care closer to home. Initial follow-up is provided through the St. Paul’s Heart Transplant Clinic, when ready, patient’s care will be transferred to the Interior Health clinic. The clinic is staffed by Nurses, Doctors, Dietitians, and other health professionals as part of the Central Okanagan Heart Function Clinic and supports patients across Interior Health.
