Social workers assess, understand and influence the social determinants of health. They address practical, emotional, social and economic barriers that prevent individuals from benefiting fully from the skilled care of other health professionals.

Social workers also help patients and clients and their families negotiate complex systems, build solid support networks, cope with stress, and strengthen or restore their ability to self-manage.
