​You can reset your logon ID and/or password if you forget your information or have attempted to log on too many times and have locked your account.

In order to reset your logon ID or password, you will be required to answer your security questions correctly. If at anytime, you require assistance, please contact MyHealthPortal Support. Follow the steps outlined below to reset your information and/or review the Logon ID/Password Reset How-To Guide.
 
Reset Login ID
 
  • Step 1: Go to Reset Logon ID form
  • Step 2: Enter email address and click Reset Logon
  • Step 3: You will receive an email with further instructions to create new logon ID
  • Note: Password and security question answers are case-sensitive
 
Reset Password
 
If you remember your logon ID, but forget your password, follow the steps below to reset just your password.
 
  • Step 1: Go to Password Reset form
  • Step 2: Fill out required fields and click Reset Password
  • Step 3: You will receive an email with further instructions to create new password

Enrol for MyHealthPortal

If you do not have a MyHealthPortal account, please request to have your email added to your patient record by emailing MyHealthPortal Support or call toll free 1-844-870-4756   (Monday to Friday, 7am – 7pm PST) – please provide detailed information, including name and phone number. Once your email has been added to your patient record, you can Request to Enrol to create your MyHealthPortal account.
